Your donation is used to purchase items that increase comfort of hospice patients, families, and caregivers.

Hospice of the Midwest is an affiliate of Brightspring Health Services. 

The mission of the BrightSpring Hospice Foundation is to provide hospice patients and their families with essential non-hospice support of which they are unable to otherwise afford. We find comfort in knowing that all patients and their families can receive assistance beyond personalized and uplifting hospice care.

The gifts provided by patrons help ensure that hospice patients and their families who seek assistance will have their end-of-life needs met beyond the limitations of hospice services. Your generous donations will help neighbors in your local community.

Gifts and support go towards:

  • Comfort care/personal items for patients with partial benefits or who are unable to pay
  • Assistance with final arrangements for those who do not have the resources
  • Assistance with home care and sitting services for those whose spouses/families must work or have no one to care for them
  • Other expenses for hospice patients in need beyond the scope of what hospice provides

The foundation supports patients and families with their hospice journey.  While the foundation does not provide health care services to patients, it does provide financial support to families in need of non-health care related assistance.

The BrightSpring Hospice Foundation is a 501(c)(3) not-for-profit organization, that serves all eligible patients in need, as such, donations made may be tax-deductible as permitted by law. Consult your tax professional for more information.

How to apply for support:

We accept applications from hospice social workers, nurses, and directors who apply to us on behalf of their patients. Applications must include an invoice or statement from the provider of the service being requested to be considered. Please note, the BrightSpring Hospice Foundation will only submit payments to service providers and does not send payments directly to the individuals requesting funds. The Foundation does not provide reimbursement for payments already made or items purchased.

To apply for support, please complete a patient/family service request form.

All applications will be reviewed with care by our board of directors to ensure the requested support falls within the guidelines of the organization.
